Terms of use

These Terms of Use ("Terms") govern your access to and use of altavidagyn.com (the "Site"), operated by Altavida Gynecology ("Altavida," "we," "us," or "our"). By using the Site, you agree to these Terms. If you don't agree with them, please don't use the Site.

These Terms cover the website only. They do not replace or modify any agreement you sign with us for care or for membership, and they do not replace our Privacy Policy or the HIPAA Notice of Privacy Practices you receive as a patient. Where a signed agreement and these Terms conflict, the signed agreement controls.

1The Site is education, not medical advice

Everything on this Site — service pages, the Journal, condition explainers, downloadable handouts — is general health education. It is not medical advice, a diagnosis, or a treatment recommendation, and it is not tailored to any individual. Clinical guidelines evolve and reasonable clinicians sometimes disagree; what's right for you depends on your history, your exam, and your goals.

Always talk with a qualified health professional about your own care. Never disregard professional medical advice, or delay seeking it, because of something you read here.

2No provider-patient relationship is created online

Reading the Site, sending a message through the contact form, or requesting an appointment does not create a provider-patient relationship. That relationship begins only when you are seen and accepted as a patient by an Altavida provider, and it is documented in your medical record.

3Contacting us and requesting appointments

• Requests, not confirmations. Booking and contact requests submitted through the Site are requests. An appointment is scheduled only once our team confirms it with you directly.
• Not a secure channel. The contact form is for general inquiries. Please don't include medical details, test results, or other sensitive health information in it. For anything clinical, call 813-733-7300 or use the secure Patient Portal.
• Accurate information. Please provide accurate contact details so we can reach you, and only submit information you have the right to share.

4Membership, pricing and insurance

Membership tiers, self-pay rates, credits and discounts described on the Site are summaries for general information. They may change, and they are always subject to the membership agreement and financial policies you receive and sign. Nothing on the Site is an offer or a guarantee of a particular price, result, or availability.

• Membership is a separate layer from your medical care. Your first visit does not require membership. To continue with Altavida on an ongoing basis, membership is required.
• Membership is not insurance. It does not replace health insurance and is not a health plan or a discount medical plan. Members remain responsible for the cost of their medical care, including copays, coinsurance and deductibles.
• Coverage varies. Whether a service is covered, and what you owe, depends on your plan and benefits. We can't guarantee coverage or payment by any insurer.

5Results and photographs

Descriptions of treatments, before-and-after imagery, testimonials and reviews reflect individual experiences. Results vary from person to person, and no particular outcome is promised or implied. Some images on the Site are stock photography used for illustration and do not depict Altavida patients or staff.

6Acceptable use

Please use the Site for its intended purpose. You agree not to:

• Use the Site for any unlawful purpose, or in a way that could damage, disable or overburden it;
• Attempt to gain unauthorized access to any part of the Site, its server, or any connected system;
• Scrape, harvest or bulk-collect content or contact information, or submit automated, spam or fraudulent form submissions;
• Introduce malware or interfere with the security features of the Site; or
• Upload or transmit anything unlawful, infringing, or that you don't have the right to share.

7Intellectual property

The Site and its contents — text, photographs, graphics, the Altavida name and logo, page design and code — are owned by Altavida Gynecology or our licensors and are protected by copyright, trademark and other laws. Product and device names referenced on the Site (for example, EmpowerRF, Morpheus8, Biote, Dysport, Restylane, Sculptra, Pro-Nox) are the trademarks of their respective owners and are used only to describe the services we provide.

You may view, download and print pages from the Site for your own personal, non-commercial use. Any other use — copying, republishing, distributing or creating derivative works — requires our written permission.

8Third-party links and services

The Site links to third-party services we don't control, including our patient portal, scheduling tools, and social media profiles. We provide those links for convenience. We aren't responsible for their content, availability, or privacy practices — their own terms and policies apply once you leave our Site.

9Availability and changes

We work to keep the Site accurate and available, but we don't guarantee that it will be uninterrupted, error-free, or current at every moment. We may change, suspend, or discontinue any part of the Site, and we may update the information on it, at any time and without notice.

10Disclaimer of warranties

To the fullest extent permitted by law, the Site and its content are provided "as is" and "as available," without warranties of any kind, whether express or implied — including implied warranties of merchantability, fitness for a particular purpose, accuracy, and non-infringement. This section applies to the Site only. It does not limit any obligation we owe you in the course of providing medical care.

11Limitation of liability

To the fullest extent permitted by law, Altavida Gynecology and its owners, providers and staff will not be liable for indirect, incidental, special, consequential or punitive damages arising out of your use of, or inability to use, the Site. Some states don't allow certain limitations, so parts of this section may not apply to you. Again, this section is about the website — it does not limit liability for the professional medical care we provide, and nothing here waives any right you cannot waive under Florida law.

12Governing law

These Terms are governed by the laws of the State of Florida, without regard to its conflict-of-laws rules. Any dispute relating to the Site will be brought in the state or federal courts located in Hillsborough or Pasco County, Florida, and you and we consent to that jurisdiction.

13Accessibility

14Changes to these Terms

We may update these Terms as the Site or our services change. When we do, we'll revise the "Last updated" date above. Continuing to use the Site after an update means you accept the revised Terms.
